This session will focus on the importance of establishing a Medical Home and Early Intervention. Families, providers, and advocates will benefit from this real world discussion on managing life, schedules and work while caring for individuals on the Autism Spectrum. This session will focus on establishing a Medical Home as a key component of Early Intervention.  Content will be delivered from a Parent perspective and from an individual who is managing 2 children’s schedules with Autism on a daily basis.  Sarah Jackson embraced this model as a single parent with 3 sons 4 and under.  Her eldest 2 sons were diagnosed 12 days a part with Autism.  This model has been an integral part of managing not only the Autism therapies and schedules, but managing life, a baby, starting a new business and volunteering in the community.

This session will cover:

  • An overview defining a Medical Home.
  • How to set-up a Medical home, manage providers and schedules.
  • Benefits and advantages for both the child, parent and family through using a Medical Home model.
  • Recommendations in managing complex system of care models for families and caregivers with multiple children of disability or those who manage individuals with multiple challenges.

This session is contributes to the community by:

  • Providing real world examples of establishing a model for managing systems of care.
  • Demonstrating a model for managing systems of care for any disability. 
  • Showing best practices in leveraging resources, creating schedules and maximizing opportunities for Early Intervention.
  • Pushing conventional thinking by encouraging caregivers to fight for the best possible care and outcomes.
  • Encouraging a model of learning through doing by lifting up your own child and lifting up others.

The discussion will also cover recommendations in selecting providers, schools and other therapies.  Further, this session will touch on the importance of managing your home schedules so you can contribute and engage in your community.  This model is supported by differentlydeveloping.com (coming Feb 2014), which will provide a platform where bloggers through the US can lead forums in their own communities.  Differentlydeveloping.com will provide a platform and voice for parents and caregivers to lead grassroots and advocacy for their own backyard.  Families, comunities and caregivers are the base that carries our cause.  The more educated and exposed familes are to information and resources, the more successful we will be in supporting individuals with Autism.
